Participant Contract / Parental Consent *Required prior to August 1, 2012-NO EXEPTIONS.
Physical Fitness / Medical History *Required prior to August 1, 2012-NO EXEPTIONS. Medical form MUST be dated after January 1, 2012.
Parent Code of Conduct *Required to be signed by BOTH parents / guardians prior to August 1, 2012-NO EXCEPTIONS.

Copy of Player's Birth Certificate *Required prior to August 1, 2012.
Wallet size photo of Player *Required prior to August 1, 2012. Photo should show shoulders and head only. Please no full body photos.
Registration Fee *Flag Football / Cub Cheer: $70.00 - All other levels, Football or Cheer: $125.00 (Checks should be made out to: Ballston Spa Pop Warner)
*PLEASE NOTE: New for 2012 Season:
ALL football players MUST be present at registration. They will be weighed and have their head measured for helmets.
